A regional health network spanning 12 acute care facilities needed a way to analyze patient population data across its entire system to support value-based care initiatives, resource planning, and chronic disease management programs. Data was locked in siloed EHR systems with no standardized schema, making cross-facility analysis practically impossible without months-long data extraction projects.
Canviz built a population health analytics platform that consolidates patient data from all 12 facilities into a unified Azure Synapse analytical store using Databricks for transformation and normalization. Clinical analysts interact with the data through role-based Power BI dashboards covering chronic condition cohorts, readmission risk scoring, and resource utilization by department. The platform was designed to meet HIPAA requirements throughout — from encrypted data pipelines to row-level security in the reporting layer.
The health network reduced the time required to produce a cross-facility population report from an average of eight weeks to under two hours. Clinical leadership identified a previously undetected high-risk diabetic cohort of 4,200 patients who were enrolled in a care management program within the first three months. The platform now supports quarterly value-based care contract reporting and annual budget planning for all 12 facilities.
